Imaging: If the appendix could not be visualized, either other views, an ultrasound and diagnostic considerations based upon WBC count and differential and physical findings could have been made. Without being able to see you and assess the situation, it is very difficult to speculate. Good luck and hope you are better soon.

Many things: That can cause pain in the right lower quadrant besides appendicitis. If can be from the colon in that area separate from the appendix, the small intestine in that area, the ovary on that side, inflamed lymph nodes or even atypical referred kidney pain. There is also an uncommon phenomenon known as epiploic appendagitis that can cause hard to diagnose abd pain. Hard to say without more info.
