Therapy or surgery: If it is a mild tendonitis it may respond to ice, nsaids and rest.. Perhaps a cortisone injection or therapy will help If there is significant AC joint narrowing, you may ultimately need an AC decompression which you certainly should try to do before you get any significant tears in the rotator cuff
