See a dentist ASAP: Toothache associated with white spots should not be ignored, because it can be a significant risk to your tooth and your health. See a dentist for examination, x-rays and treatment. Do not delay, bad tooth ache will not go away on its own.

Infection: Probability is that you have infection of tooth/teeth, gums, or both. Can't diagnose or treat this over the computer. This is not a DIY problem. Do what you would do if you had a severe infection anywhere else in your body, seek immediate professional help. Call your Dentist for an emergency appointment. Don't put it off.
