Doubt: It should be a very unusual situation,......use of coccain, congenital CAD, very bad DM/HTN/cardiomyopathy etc.

SPECT vs. PET: On regular myocardial perfusion SPECT imaging, large breasts and large body habitus can cause attenuation artifacts. This is often avoided on cardiac PET where attenuation correction is used. If the breast shifts between stress and rest SPECT imaging, false positive for ischemia could occur.
