Go to see MD: Your grandmonther's blood pressure is way too high. She needs to be seen ASAP by an MD to get better 24 hours control. Does not need to be brought down to 130/80 or less as in a young person as she may not do well with such a low relative BP for her; but atleast down to 160/90 mmHg for now. Then reassess. She needs to be seen by MD ASAP.
