Stop smoking: Do the obvious. Try to quit smoking and start brushing your teeth twice a day, floss daily and use an anticavity mouth rinse. I would encourage at least 2 visits a year to the dentist for eval and cleaning

Evaluation: You can have yourself evaluated by a local dentist. Follow treatment advice and bring your oral health back into good shape. Stop smoking, brush and floss thoroughly, and visit your dentist every 4 to 6 months for maintenance. That's a great place to start...

Professional: See Dentist to asses the extent of your gum disease/bone loss. Referral to gum specialist, Periodontist, may be in order. Smoking only makes things worse (but you already know all the bad things smoking will do, including damaging your gums).
