Irregular heart beat: In general, you want to start with a primary care physician (family physician or internist) and have them decide whether referral to a cardiologist is necessary. They can do an ECG for you, which sometimes will give an answer to your situation right away.

Primary care: One should start with a primary care provider in family practice or internal medicine. They can do the initial workup with labs, ekg, holter monitor and possibly other studies and begin therapy. Then if they feel it is appropriate, they can refer to a cardiologist.
