Middle: The ascending aorta is in the middle but is slightly shifted to the right . The pain associated with dissection of the ascending aorta is to the front of the chest,very intense and diffuse rather than in a discrete area.In contrast dissection of the descending arta is associated with pain in the back

Yes: The aortic root in general arise from the the upper part of the heart and goes up and turns toward the back of the chest and goes down the back of the heart and when you have a dissection you will feel pain between the shoulder blade. Depending on where the dissection is determine how high or low the pain is in the upper back. The pain can be quite intense. This is a medical emergency.
