4-12 weeks: Current guidelines recommend a TSH below 2.5 in women trying to conceive and those in the first trimester. 25 mcg daily of levothyroxine may achieve that goal in 4-6 weeks but the TSH should be repeated to confirm prior to conception. If it is still over 2.5, then the dose may need to be increased and another TSH repeated in another 4-6 weeks.
