Minimal: The thyroid gland lives near the jugular vein and carotid artery but those are rarely at risk. Generally total thyroidectomy with I-131 is curative most of the time. The most common risks are to the voice box nerves and calcium glands next to the thyroid. Long term surveillance is appropriate.
