Severe memory loss: with ataxia can be seen with B12 deficiency. This can also cause neuropathy in addition to brain and spinal cord degeneration. Vit D deficiency may contribute to memory loss. Fever....probably not.

Vitamin D: A vitamin D level that low is very uncommon. I would have your doctor prescribe 50,000 units omnce a week for two weeks and then have your vitamin D level measured again.
