Muscle pain: Likely to be muscle pain from spasm. You may benefit from local heat, massage, manipulation therapy. Do rule out nerve problem - do you have any pain radiating down your leg?. Hope that helps.

Pain in back: This pain seems muscular, not internal - particularly since your pain worsens with tilting head forward. Apply heat, stretch, relax.
