Most likely: Given the normal stress test, the likelihood of cardiac disease is low. If the area is tender, it is more likely to be musculoskeletal pain. That being said, your troponin is a little worrisome (depending on what is an abnormal troponin at your hospital). I'm sure they set you up to follow up with a cardiologist. I would keep that appointment. If they didn't, call one just to be sure. Better safe.

Likely: Cardiac pain will not be associated with external tenderness. This sounds like costochondritis. A good history and physical should be able to determine this.
