RV echo: The echo appearance of the right ventricle is very position sensitive. Without seeing the actual study, the measurement isn't that informative. The 48mm if at the right angle could be normal; if it is a direct diameter from the IV septum to the free wall of the RV through the short axis of the heart, it could be quite dilated. You need to ask your Doctor the meaning of this measurement in you.
