Always: Start with the least invasive procedures. Surgery should be your last option. Try everything you can before going that route.

Neither of the above: Simple and conservative is a better approach. Before either of your suggestions, physical therapy is a logical place to start. If that fails, more aggressive treatment can be used. Surgery is used only if the problem persists and other methods have failed. See your physician to get started.
