Recent ultrasound says ivc remains prominent in size sometimes with cardiac decompensation what does this mean?

Cardiologist: A prominent Inferior vena cava on ultrasound can be seen with right-sided heart failure. You should talk with your doctor about the results and consider an echocardiogram if there is not another reason for your inferior vena cava to be prominent.

"sometimes" odd.: The inferior vena cava (is one of 2 major veins) that returns deoxygenated blood to the heart from the systemic circulation. If it is dilated that may be a consequence of increased right-heart pressures, typically from a leaky right sided valve. "Prominence" alone does not signify pathology. And "cardiac decompensation" is an odd choice of words. Get the report. Clarify with a cardiologist.
