Many things: The list of causes is huge with just that symptom and includes appendicitis, ovarian or pregnancy issue, simple gastroenteritis, or even muscular pain. It is important to know if there are any associated symptoms such as fever, diarrhea, what makes pain worse/better, or irregular periods.
