Many things: The list of causes is huge with just that symptom and includes appendicitis, ovarian or pregnancy issue, simple gastroenteritis, or even muscular pain. It is important to know if there are any associated symptoms such as fever, diarrhea, what makes pain worse/better, or irregular periods.

Various causes: Right lower quadrant abdominal pain can be caused by various mechanisms, including: Pancreatitis-periumbilical pain, appendicitis - pain at McBurney's point, kidney stone, ovarian cyst, cystitis due to urinary tract infection, biliary colic due to gallstone or common bile duct abnormality or obstruction, Psoas muscle strain, gastroenteritis. It is always good idea to consult your physician!
