Appendix or: The bowels are in that area so could be constipation, cramp, the appendix is there so it can be inflammation of the appendix, and there is of course the bladder more to the middle of that. Might be an ascending urinary tract infection? Hard to say without more symptoms/details: nausea? fever? pain with urination?
