Get going: If you have a history of an internal knee injury, and the symptoms return, you need an examination and review of your care. If you have gained 15# after your last knee injury, your gait has changed, and your physician's recommendations may change accordingly. Physical Therapy may be helpful. Good luck.

You have crepitus: It sounds like you have crepitus, and from what you write, it may be that the undersurface of your kneecap (patella) is rubbing on the front of your thigh bone (femur). You may be experiencing what is known as patellofemoral syndrome. Your doctor can either show you exercises to correct this, or send you to a physical therapist who can. With good care you will be able to prevent needing surgery.
