First see: First see a dentist. I think most if not all your symptoms are dental in origin. If not your dentist will recommend that you see your physician. BTW you don't have to see something in order to have a problem. May be bruxism/TMJ issue.

See: See both. May be TMJ, ear/sinus/gum/tooth-teeth infection, viral or bacterial infection. Call both for examinations.
