Depends: Usually cysts smaller than 5 mm there are no worry. At the size you mention, this is large and may require intervention if there is compression of adjacent structure or the aqueduct.best to discuss with your doctor.

Calm down: Pineal gland cysts are more easily seen in MRI studies than in CT studies. So the cyst may have been present in the CT study as well, but not well seen. A pineal gland cyst by itself is usually not important. Discuss your concerns with your Doctor, and have your Doctor ask the Radiologist to compare both studies to see if there are any significant changes.
