Highly variable: The biggest risk in coming off a maintenance drug for opioid addiction is NOT the harm to the fetus as much as it is the harm to the mother - relapse. There are no controlled trials of this, but the relapse rate of women who come off methadone is over 90%, and the use of street drugs, the crime necessary to obtain them, the adulterants and contaminants and lifestyle issues are what is harmful.
