Many reasons : There are many reasons for enlarged lymph nodes. Not all are infected. Such entities as hydradenitis and inflammatory causes can enlarge the node as well as yes lymphoma. These may not respond to antibiotic treatment. You should talk to your doctor as to what may be going on.

Abnormal?: Were the mammogram and ultrasound abnormal? If so then I would assume you are set to see a breast specialist for biopsy? The lymph node is coming from whatever process (tumor) is going on in the breast.
