23 weeks and doctor says a small amount of fluid around baby's heart....is this because i smoked cigs up to my 4 month? (i was unaware i was pregnant)

Smoking: There is no way that anyone could say definitively that smoking caused any particular case of fetal pericardial effusion. That being said, I encourage you to continue to not smoke cigarettes, both for your health and the health of your baby.
