Get to the hospital: This is an urgent situation and it requires an immediate trip to the emergency room to have the cast removed. If a cast is too tight it can damage blood vessels and create permanent damage. Nerves can also be injured and they don't take kindly to those types of insults.

Call your doctor: You need to check with the provider that placed the cast and determine if bivalving (splitting it down the sides) is needed.
