Type 1 or Type 2: The major question is if he has Type 1 or Type 2 diabetes. It sounds like he is young. If he is not significantly overweight and has no family history of diabetes, he may be an early Type 1 diabetic. In that case metformin will not help. He will need to be on insulin eventuallly. If he is overweight, he may be Type 2, and losing weight may improve his diabetes so that he does not need metformin

Control: Hi. Type 2 diabetes (assume) and high cholesterol both increase heart attack risk and diabetes causes eye, kidney, & nerve risks. Unless he's one of the VERY, VERY RARE people who can control it VERY WELL with diet & exercise, he'll need diabetes medicine for life (metformin and/or others; 1 drug alone seldom adequate for long). Statin drug too for cholesterol. Healthy diet & exercise FOR LIFE.
