Toilet training: Sacral dimples are not unusual. If you can see where it ends, there is no concern. When you can't, then an evaluation for spinabifida is indicated. This is usually done at birth. 3 years is still within normal for potty training. Don't make it a battle. Doubt: Your pediatrician should have picked that one up a long te ago if serious. Ask your pediatrician if concerned. Not abnormal not to be completely potty trained for bowels yet

Get ultrasound: He is too young to be concerned about bowel training difficulty. You can speak to your pediatrician about an ultrasound of the sacral area or imaging. If the clinical exam by his pediatrician shown normal rectal tone, probably not a problem.
