Consider: Possible inflammatory problem, or maybe a nerve issue involving small fibres, which will not be picked up by your prior studies. Did EMG study brachial plexus?? Were blood tests helpful? Did you see a rheumatologist or a neurologist. Does Fabry's disease run in your family? A few random thots, to run by your doctors.
