By the symptoms you describe and the works-up you have gone through, I assume you have been thoroughly evaluated by a neurologist. If you do not have a satisfied answer to your condition, you should seek a second opinion with a neurology team at a tertiary, academic center. Good luck.

In order to have the best success with physical therapy, one needs to find the precise "pain generator". The EMG study and the MRI exclude only some of the possibilities. Problems in the brachial plexus (such as thoracic outlet syndrome) need consideration. Obtain a consult from a vascular surgeon, and seek a further answer.
