Not required.: Draining can help relieve pain but it is not required. two methods to punch hole in nail: #1. drill a hole with a sharp object such as a hollow needle or small drill bit #2 uncoil a paperclip half way. bend clip to form a 90 degree angle 1/16 inch from free tip. Heat to red hot and poke short tip through nail as quickly as possible. Instant relief. I am sure there is a youtube.

Subungual hematoma: You do not necessarily need to have it drained unless it remains painful, as draining the blood is mainly for pain relief. If there is a greenish colour then that suggests that the blood is beginning to be broken down and reabsorbed. There is a chance that the nail may loosen and detach, but if that happens a new, healthy nail will grow out in its place if the matrix of the nail bed is uninjured.

Drain it: There maybe an infection present. I would suggest seeing a podiatrist to drain the nail and Rx antibiotics. After the nail is removed, the doctor can check the nail bed to see if there is any indication of a fracture.
