No : No test can ever guarantee that anything is absolutely 100% normal. There is always a margin of error, however small. 2d echo is good for detecting structural and motion abnormalities of the heart. For other abnormalities, like coronary artery disease, different tests are needed. So the question of whether you need to go further than an echo really depends on what your doctor is looking for.
