Not necessarily: Pyelonephritis is an infection involving the kidneys. Typical presentation is a high, spiking fever that often is accompanied by flank pain. It is not uncommon to have continued fevers even on antibiotic therapy. It is very important to have antibiotic therapy tailored to urine culture data to optimize treatment. Back to ER if uncontrolled nausea, lower blood pressure, worse pain

Does kidney infection always have fever. ? I am on bactrim (sulfamethoxazole and trimethoprim) for UTI for 2 weeks. Still having some urgency pain and now have periodic back pain.
Lower abdomen pain, but also flank pain (soreness) on right side, and right lower back soreness, very low near pelvic bone. No fever and UTI test negative. Ovarian cyst possibly? Something else? Still have kidney infection if uti negative?
