D dimer test 550, high red blood count, long term shortness of breath for 5 years. what could be problem?

Context: Is the d dimer normal or high for the lab? Is there a history of smoking cigarettes, or a history of blood clots or emphysema? Chest pain, coughing up blood? This sounds like lung problems, but could also indicate a blood problem. More clinical info is needed.
