Very quickly: Scarlet fever, the triad of fever, strep throat & scarletina rash, is treated very soon after starting penicillin antibiotics (assuming no allergy to this medication). Within 24 hrs of starting antibiotics patients are no longer infectious to others. The rash may last for several days or even a week, but fever & sore throat improve much sooner.
