Speculative: I understand you have idiopathic thrombocytopenic purpura and a pilonidal sinus. I trust you're getting the latter managed properly, these can be very unpleasant. Does pain near tailbone with a pilonidal sinus always indicate that there's an infection? Or could the sinus hurt even without being infected? Also, I don't feel any painful bump, just the sinus. No pus drainage as far as I can tell?
