Child with fever and cough. on antibiotics but looking like it's not effective, so maybe change medicine?

Probably viral: Children with cough and fever often do not have a bacterial infection, the only thing that antibiotics will be effective against. If that is the case, then changing antibiotics will only subject them to the risks associated with inappropriate antibiotic usage. If they are not very ill, let it run its course. They will develop resistance to that one of hundreds of viruses which may affect child.

Be patient: Most cough/fever in a kid is a cold or some other virus. Treating a cold with antibiotics will not heal it faster or fix a fever that is not caused by bacteria. If the kid was labeled as a bronchitis or some other infection, the antibiotics may or may not have been needed. Unless the kid is going down hill you should give it some time to self heal.
