Yes: You need to discuss it with your physician very soon & likely needs prompt care with cultures & further studies. Could be a higher dosage or a different antibiotic. It can also be a non infectious cause.

Not necessarily: The most likely explanation is that the fever and cough are being caused by one of the many respiratory viruses that are very common this time of year. Antibiotics will not be effective because there is no bacterial infection. It should run its course, but if the fever persists for several more days it is important to have him re-evaluated by his doctor.
