It is impossible: " 3 white skinny things out of his tooth" are food particles and not worms. This condition you have described confirms that tooth decay is advanced. The best course of diagnosing tooth decay is through routine visits to your dentist. Your friend should call his dentist today for examination and treatment, before is too late saving the tooth.

Worms: In the 16th and 17th century tooth decay was thought to be caused by worms. We now know it's caused by (microscopic) bacteria. "White skinny things" were probably food debris. Decayed (infected) teeth do not self-cure. Your friend needs to seek out dental care before the spreading infection does severe harm to other organ systems.
