Fever and back pain: Yes, you should see a doctor. Symptoms of fever, chills, back pain could be related to urinary tract infection.

Fever smelly pee: Yes. These symptoms could be just a mild cold or illness but this combination is also very concerning for a possible upper urinary tract infection or kidney infection. These can be usually treated very easily with antibiotics on an outpatient basis if treated early enough but if treatment is delayed you could have serious complications. Please go to a doctor or ER right away for evaluation.
