Sinus: It could be allergies. You may want to do allergy testing. It could also be secondary to food sensitivities so consider and elimination diet. Occasionally after long term treatment with antibiotic you can get fungal infection of the sinuses. Full evaluation by an ENT would be important. Dizziness could be unrelated or side effect of medications. Ear pain can have other causes such as TMJ.
