It can do both: the metabolism of tobacco smoke involves certain liver enzymes, and genetic differences in these enzymes may result in why smoking is more toxic in some people. In addition, the lung contains certain enzymes that combat the damage of smoking, and some people have less of these enzymes, also increasing potential for damage. Same seems to be true with heart disease and smoking.
