Possible Fracture ?: It is very possible you fractured a tooth if you bit down hard... But this is only a guess since i cannot examine you. Please see a local dentist for an evaluation.

TMJ: The most likely diagnosis based on your history is a subluxation or shifting of your temporomandibular joint that connects your upper and lower jaw bones together just below your ear. Since you are still having difficulty chewing with a feeling of misalignment of your teeth i recommend you make an appointment with your dentist.

Disc: There is a cartilaginous disc between the upper and lower boney parts of your temporomandibular joint. Sounds like it's out of position. Please call your dentist for help...The disc has to be recaptured.

Muscle Spasm: You probably grit your teeth when you swing the club. Sounds like a spasm of your Masseter (cheek) and Temporalis (temple) muscles. Soft diet, warm compresses and avoidance of stress should help in a few days or a week. Golf is supposed to be fun. Try to relax!
