Severe reflux?: You need to discuss this one with your own physician, who will know your history a lot better than we do. You probably need upper GI endoscopy to determine the extent of regurgitation and possible damage to your esophagus. If severe enough -- as in barrett's esophagus -- you could have some cancer risk.

Procedure: Cancer risk could only be judged by endoscopy a look in esophagus with lighted instrument under mild sedation to look for premalignant spots in esophagus.

Yes need scope: It is the presence of reflux more than the symptoms that increase the risk of developing barrett's esophagus. Barrett's esophagus is associated with an increased risk of esophageal cancer and needs surveillance. So with symptoms like you have you should see a G.I. Doctor and have and egd upper-scoop.
