Many: Add a long-acting broncholator or singulair (montelukast) to qvar. If still not responding, xolair ( a hybrid antibody to bind the allergy antibody) may be tried. Spiriva has bronchodilator effect nearly as good as bronchodilator. See an allergist to find out if there are allergen triggers that can be avoided.

Singular : This might help. Please discuss with your doctor.

See below: There are many. You may need to add a long acting beta agonist. Also, I would have your IgE level and an allergen panel done. You might need xolair and/or zyflo. I would discuss your concerns with a pulmonologist.
