Difficult to know: I can imagine that a scar that is high in the vagina would be easy to miss or easily overlooked during an exam. And almost impossible for your partner to feel during sex. If it's not bothering you, then i wouldn't pursue it. If you are worried about misdiagnosis, then you should bring it up during the exam and have your gyn explain what s/he is feeling.

No : Not quite sure what you're describing but it wouldn't be picked up on a pap test. If your pap was hpv pos in the past and now it's normal it just means your body fought it off on it's own.
