Scratch: As long as it only a scratch just clean the scratch and apply an antibiotic ointment to the source.

Skin abrasion: Ok to do local wound care with gentle soap and water cleansing for now and possibly apply an antibiotic ointment. Tetanus shot should not be needed, especially if your baby got routine vaccinations at two months. Take your baby to his/her doctor if the area becomes red, swollen or develops a discharge.
