No: Fever is not always present in appendicitis. Symptoms typically start with generalized abdominal discomfort, constipation, and pain around the belly button. Over 24-48 hrs, the pain typical moves down to the right, lower abdomen and becomes more severe (hurts to walk or go over bumps in the car). At this point, the pain may be associated with: nausea, vomiting, fever, and chills.
