Surgery: A sizable hematoma that has been persistent for that long could certainly cause significant symptoms. I would ask your physician whether surgical evacuation of the hematoma would be an option for your situation. Sometimes simply removing the hematoma can result in dramatic pain relief, provided that this is an option in your particular case.

Hematoma Lipoma: You really need to see your local primary care physician for further evaluation and management or referral for management. If fluid is still there, it needs to be aspirated. If there is excess skin, it should be excised. And if there is a lipoma, you could have that removed as well.
