Watch & Wait: Pulling at the ear doesn't necessarily mean anything. Some kids will do it if they have fluid behind their eardrums & no infection; some do it for no obvious reason at all. If he's not running a fever & showing no sign of distress, just keep giving him the antibiotic & don't worry about it.

See your doctor: Ear infections that have not improved clinically in 48-72 hours on antibiotics are by definition an antibiotic failure. The child needs to be re-evaluated and perhaps have the antibiotics changed.
