Holding his : Ear does not necessarily mean infection . Let him discover his ears. Most will have fever, tender to touch, fussy , irritable, drainage at times.Sucking a pacifier will help with earache. If somewhat in pain you can certainly give some tylenol (acetaminophen).If no better and now with symptoms above , get it checked - he may indeed have an ear infection!

Ear issue: He may hold his ear for many different reasons such as playing with it. As long as he doesn't have fever, difficulty breathing or other concerning symptoms, i would not worry.
